Home Health Registered Nurse

Requirements:
We are hiring for field nurses in the Lake County area. We understand flexibility in your schedule is important to you. As a Home Health Care RN with Northwest Home Health & Rehab, your visits are tailored to your geographical needs and schedule.
Qualifications:
o Registered Nurse licensed in the state of Illinois
o Nursing experience in home health or ICU
o Effective written and verbal communication skills
o Strong desire to help others
o Experience in specialty areas such as IV and wound care or a psych certification are a plus but not required
Responsibilities:
o Monitoring, recording, and reporting symptoms or changes in patients' conditions
o Recording patients' medical information and vital signs
o Modifying patient treatment plans in conjunction with physician input as indicated by patients' responses and conditions
o Maintaining accurate, detailed reports and records
o Consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans
o Monitoring all aspects of patient care, including diet and physical activityEstimated Salary: $20 to $28 per hour based on qualifications.
